Complete Required Medical Forms: As part of the registration process, we kindly ask you to download and complete the following medical forms. These forms are crucial for the well-being of your child during the camp. Please submit these completed documents at the check-in, as we are bound by strict Department of Health regulations.

• Camper Health History Form

• Camper Permission to Treat Form

Important Note: No child will be permitted to participate in the camp without the submission of these completed documents to the Camp Director.

1: Print Documents: First off, print the documents sent via email.

2: Complete Forms: Fill in the necessary details on the forms. Accuracy is key!

3: Email Submission: Once the forms are all squared away, shoot an email to camps@mdssoccer.com. Remember, timing matters—aim to get this done at least 2 weeks before the camp kicks off.
